Crestwyn behavior health treat staff very poorly. In Crestwyn everyone in the building was related and nothing was ever done fairly. Human resources never addressed any of the problems brought to them about staff. The CEO herself wasn't pleasurable either. I wouldn't refer anyone to work and take their loved ones there, emergency psychiatric condition don’t exist. Any deficiency statement ending with an asterisk (*) denotes a deficiency which the institution may be excused from correcting providing it is determined that other safeguards provide sufficient protection to the Patients. the hospital failed to ensure patients received care in a safe setting to protect their emotional health The findings included: Review of "Patient Rights" revealed, "Patients have the right to be treated with consideration, respect and full recognition ,,,There was no documentation of personal items such as clothing in Patients items They did not have a personal Hygiene product facility failed to ensure medical record are correct crisis/chief complaint Review of the Incident Reporting Form for Incident #1 revealed and verifying the employee continued to work after the witnessed incident of verbal abuse. To the cno Michael roper The facility failed to ensure all patients were free of abuse. The facility failed to ensure the incident was immediately reported and the patient was immediately protected from further abuse during the investigation and the cno Michael was told about a man beating on old lady’s daily and did nothing the ceo knew about it and the HR The facility failed to conduct a thorough investigation that identified the correct date the incident occurred. The incident investigation and conclusion were not completed. I reported, investigated and protected the patient(s) from abuse. Documented the following statements: the employees sleep all night while men went in to women’s room and the staff ruffled the children up bad sometime leaving bruises and cuts on their body’s and Michael roper lets the patient’s walk around with feces and urine because he does not want to supply new scrubs are socks and they treat the white patients horrible also the ceo lets them smoke weed because she claims could smell , but she never leaves her office hmmm just look up why the last cno and ceo and risk management got fired. And don’t forget the nails and long sharp pieces of metal all ways falling off the day room doors and spiders crawling from resident ear and cockroaches everywhere and flys also the vitals machine never works and dangerous people jump over the fence all the time.
